STEP 1

Fill, sign and submit this digital service agreement form to Enabled4Life’s national intake team for processing.

Receive your welcome pack via email

STEP 2

Call the NDIS National Contact Centre on 1800 800 110 and nominate Enabled4Life as your “plan manager” and “my provider”.

Receive your budget app login details in your email and begin the process of submitting provider invoices for payment. If you have any questions, feel free to call 1300 667 454 to talk with a member of our team.


PACE Plan Nomination Process

The Quickest Way to Nominate Enabled4Life

After submitting your service agreement, simply contact the NDIS National Contact Centre on 1800 800 110 (available 8am–8pm local time) and nominate Enabled4Life as your plan manager and my provider.

When speaking with the NDIA Service Officer, provide them with the following details:

  • Legal name: ENABLED4LIFE ENTERPRISES PTY LTD
  • Business name: Enabled4Life NDIS Plan Management
  • ABN: 9163615930
  • NDIS Provider Number: 4050073162

The nomination process should take effect immediately, allowing the Participant’s plan to appear in Enabled4Life’s PACE portal without delay.
