STEP 1
Fill, sign and submit this digital service agreement form to Enabled4Life’s national intake team for processing.
Receive your welcome pack via email
STEP 2
Call the NDIS National Contact Centre on 1800 800 110 and nominate Enabled4Life as your “plan manager” and “my provider”.

Receive your budget app login details in your email and begin the process of submitting provider invoices for payment. If you have any questions, feel free to call 1300 667 454 to talk with a member of our team.

The Simplest Way To Nominate Enabled4Life
Please call the NDIS National Contact Centre on 1800 800 110 (available 8am–8pm local time) and nominate Enabled4Life as your plan manager and my provider.
When making the request, please ensure the NDIA Service Officer uses the correct Provider Registration information as outlined below:

The nomination process should take effect immediately, allowing the Participant’s plan to appear in Enabled4Life’s PACE portal without delay.
